Medication Management
- Assistance with ordering prescriptions, medication reminders, administration, and ensuring proper usage.
Household Support
- Support with cleaning, cooking, laundry, and maintaining a safe and comfortable living environment.
Meal Planning and Preparation
- Support with meal planning, grocery shopping, and cooking, ensuring nutritional needs are met.
Companionship and Social Support
- Encouraging socialization and participation in community activities, fostering emotional well-being and reducing isolation.
Transportation Assistance
- Providing or arranging transportation for medical appointments, errands, social events, or community engagement.
Budgeting and Financial Support
- Assistance with managing finances, paying bills, and budgeting to ensure financial independence.
Health and Wellness Monitoring
- Regular check-ins to monitor physical and mental health, including coordinating with healthcare providers.
Behavioural Support
- Implementing strategies and interventions for individuals with behavioural or mental health challenges. We work alongside other professionals such as social workers to formulate support plans which best support each young person.
Life Skills Coaching
- Offering guidance and training in skills like cooking, shopping, time management, and communication to enhance independence.
Crisis Intervention
- Providing immediate support during crises, ensuring safety and well-being during challenging situations.
Employment Support
- Helping individuals prepare for, find, and maintain employment, including job coaching and workplace accommodations.
Support Accessing Education
- Encouraging and facilitating participation in education services.
24/7 On-call Support
- Providing emergency assistance and availability outside of regular service hours for urgent needs.
These services can be tailored to meet the specific needs of each young person, with our main goal being to build independence and prepare them for life beyond supported living.
We offer a variety of different support levels ranging from 1:1 and 2:1 support through to minimal support.
